Guide to Completing Form PSC2

Please type wherever possible. If filling in the form manually remember to use black ink and please write clearly.

 

 Form Field  Guidance
 IMO Number
(your own vessel) 		Fishing vessels not assigned an IMO number should provide their External Registration number.

Please do not give your External Registration Number if you have been assigned an IMO number.
Radio Call Sign
(your own vessel) 		This is essential information. You must include it even if you have already given both the vessel name and the IMO.
 Flag State
(your own vessel) 		Use FAO three letter code
 Email address:  It is essential that you include as full contact details as possible
 Phone number:
 Fax number:
 Port of Landing or Transhipment  Please give Port and Country
 Date  Please use format DD/MM/YY
 Catch Information of Donor Vessels  Fill out one Form PSC2 for Each Donor Vessel
 Name of Vessel

(Donor Vessel) 		 This is essential information

Area of Catch
(Donor Vessel Catches) 		 NEAFC Convention Area (ICES subareas and divisions) NAFO RA (sub division) or Other (EEZ)
 Total catch onboard  
The total catch onboard your vessel from the donor vessel named on your PSC2 form
 Catch to be landed  The catch to be landed from the donor vessel named on your PSC2 form
  Species  Use FAO 3 letter code as set out in Annex V of NEAFC Scheme of Control and Enforcement.
 Product Use FAO Product Codes List as described in Appendix 1a to Annex IV of the NEAFC Scheme of Control and Enforcement
